Alzheimer's Society's Dementia Adviser Service

The Dementia Adviser Service is available to people who have received a diagnosis of dementia, or are in the process of receiving a diagnosis.
Dementia Advisers provide support to understand key information around a diagnosis of dementia and what this may mean for the individual and their family. Information can be requested which might be useful to carers, family members and friends.
When more intensive, group, or specialist support is needed, dementia advisers will refer or signpost to other available support and services.
The Herefordshire Dementia Adviser Service is a partnership with 2gether NHS Foundation Trust. A named 2gether Community Dementia Nurse and a named Alzheimer’s Society Dementia Adviser work together in each of the three areas; North, South and East Herefordshire. Each partnership is responsible for covering all GP surgeries within their area and holding a joint clinic in each practice.
There is also a countywide Alzheimer’s Society Dementia Adviser to support younger adults with a diagnosis. Support is offered in both clinics and/or the person’s home.
Every person who receives a diagnosis is allocated a named Dementia Adviser who is a main point of contact.
