Community Connectors

Our Connectors can support individuals to get involved with local groups and organisations, making the most of the fantastic support already available nearby. Anyone can refer into the community connectors whether that be the individual looking for support in their local area or you’re a professional working with members of the public.

If you, or someone  you know, could benefit from local support, whether it's help accessing services, reducing isolation, or getting involved in community activities, please visit our referral page or scan the QR code. Please note: referrals will only be accepted from anyone aged over the age of 18 and living in Herefordshire.

Sometimes people don’t know where to turn for help until things get really tough. That’s where Community Connectors come in! They help people find support early on, bring neighbours together, and make sure no one feels alone. It’s all about building stronger, more connected communities where help is easy to find and close to home.

Community Connectors are all about people — listening, supporting, and helping make connections that matter.

  • They chat with people about what really matters to them, focusing on strengths and what’s going well.
  • They help folks build confidence, stay independent, and feel more connected.
  • They spot what’s missing in the community and feed this back to our development team.

Community Connectors reach across the county and can support people in their local community.

Information Held About you

To provide our community connection services to you, it is necessary for us to collect and hold personal information about you. This information will include: -

• Name, postcode, age range, contact details
• Information from interactions we have with you, this could include a telephone call or face to face meeting and the service we think may best support you.
• We may collect a wellness score to measure the impact of our service.

Who is processing my data?

All personal data held, is processed in accordance with data protection law. The Data Controller for the information outlined in this privacy notice is Herefordshire Council.

How will we use the Information we hold about you?

We will collect information about you (where applicable) to: 
• To contact you to offer signposting support and further support if needed.
• To assess if there any areas of need which are not being met in your local community.
• To contact you to review if our support has been beneficial to you.

What is the legal basis for us to process your data?

The legal basis for processing the data is your consent, which you can withdraw at any time by notifying us at talkcommunitychamption@herefordshire.gov.uk

Who we will share your information with

We will not normally share your information with organisations other than our partner organisations without your consent. However, there may be certain circumstances where we would share without consent such as where we are required to do so by law, to safeguard public safety, and in risk of harm or emergency situations.

Any information which is shared, will only be shared on a need-to-know basis, with appropriate individuals. Only the minimum information for the purpose will be shared.

How Long do we Keep your Records?

We will only keep your information for the minimum period necessary. The information outlined in this privacy notice will be kept for 3 years. All information will be held securely and destroyed under confidential conditions.

Your rights

You have several rights under data protection law, including the right to request your information and to request that the information be amended or erased if incorrect.

To request your records, you will need to put your request in writing and provide proof of identification to the Information Governance Team, Herefordshire Council, Plough Lane, Hereford HR4 0LE or via email, informationgovernance@herefordshire.gov.uk
